We Will Self Certify you to the State of Minnesota and save you a trip to the DMVI found a driver today whose urinalysis tested positive for sugar in the urine, which is a serious finding and requires further evaluation. He must go to his own physician and request to be evaluated for glycosuria.
In my opinion the most appropriate testing is a blood test called glycosylated hemoglobin or hemoglobin A-1 C.
For those who do not
have a physician and /or are uninsured I can arrange that test for you through
LabCorp. They have labs to draw blood in
Edina and Bloomington. The cost for the
lab test and the follow up office visit to review the results is $70.00 and
must be prepaid.
The following is the Wikipedia definition of Glycosuria or glucosuria:
“is the excretion of glucose into the urine. Ordinarily, urine contains no
glucose because the kidneys are able to reclaim all of the filtered glucose
back into the bloodstream. Glycosuria
is nearly always caused by elevated blood glucose levels, most commonly
due to untreated diabetes mellitus.”

A normal A1 C. level
can range from 4.5 to 6% for someone who has no abnormality of blood sugar
metabolism. Levels above 8% indicate
prolonged untreated diabetes.
A1C levels of 6.5%
or higher in two separate tests will be the diagnosis of diabetes. Pre-diabetes is diagnosed when the results
are between 5.7 and 6.4% on two separate occasions. This indicates you have a very high risk for
developing diabetes. If the blood test
indicates diabetes you will need to be treated by a medical doctor before I can
issue a health card. If you are pre-diabetic,
you will not need treatment. However,
your health card will be issued for one year at a time to monitor your health
status closely.

No carrier shall require or permit an unqualified driver to operate a
commercial motor vehicle. A carrier in Minnesota is subject to the rules for
driver qualifications if it operates vehicles that are:
• Of any size operating in intrastate commerce as a for-hire property
carrier or transporting solid waste as described in Minn. Stat. §
221.025, clause (2);
The Term for-hire is vague, what does it mean?
A for-hire motor carrier means a person engaged in the transportation of goods or passengers for any kind of compensation: promised, paid, or given.

Dr Jim Seim is one of the first in the State of Minnesota to become a Registered Medical Examiner on the Federal Motor Carrier Safety Administration's National Registry of Certified Medical Examiners. He has completed the required training and testing concerning the Federal physical qualifications and standards for truck and bus drivers and possesses the necessary knowledge and professional credentials to perform physical qualification examiners for commercial motor vehicle drivers in accordance with the Federal Motor Carrier Safety Regulations ( 49 CFR 391.41 -391.49).

A Department of Transportation (DOT) physical examination is conducted by a licensed "medical examiner." The term includes, but is not limited to, doctors of chiropractic (DC). Dr. Jim Seim has passed the National Registry of Certified Medical Examiners Exam further he is licensed by the State of Minnesota to perform Department of Transportation (DOT) physical exams and issue DOT Medical Examiner Certificates. Which may also be known as a MN Health Card but valid in all 50 states. This is a convenient location for your MN Dot physical examination.
The $70.00 charge includes the DOT physical examination, visual acuity, hearing tests, urinalysis (blood, sugar and protein) and a laminated Health Card. The urinalysis test is not a drug test. The DOT Health Card will be completed at the end of your DOT physical exam. The MN health card is valid in all 50 states.
We are located at 20010 75th Avenue North in Corcoran, Minnesota near the intersections of County Road 10 (Bass Lake Road) and County Road 116. There is convenient truck parking. See below for directions.
We make every effort for same or next business day appointments. We also will track your DOT health cards expiration date and notify you when the DOT Medical Certificate expires. What to Expect on your visit
While we make every effort to accommodate walk-ins those with appointments will be given a priority. We have all of the necessary forms that are required You need only bring a valid drivers license, and be prepared for the urine test. The DOT physical examination takes approximately 20 to 30 minutes.
You will be examined according to the Federal regulations that are listed on the bottom of this page but you may expect the DOT Physical to include: Vision, traffic signal colors, blood pressure, pulse, eyes movements, ears, peripheral vision, back of the mouth, neck lymph nodes, spine for tenderness, spasm, range of motion, abdomen for tenderness, hearing, reflexes, balance, neurological checks and a hernia check.
We will complete your Medical Examination Report for Commercial Driver Fitness Determination form, give you a summary of your exam and provide a Medical Examiner's Certificate which is commonly known at the DOT Health Card or a MN Health Card as appropriate.

What to know before your visit
While may people tell us this is one of the best physicals they have had it is NOT intended to substitute for routine health evaluations by your primary care provider. We can not prescribe or refill prescriptions.
